There are two separate application forms and two separate fees for the Pharmacists Document Evaluation and the Pharmacists Evaluating Examination. The fee for the Pharmacists Document Evaluation is CAD$515 until May 31, 2009. Effective June 1, 2009 the fee for the Pharmacists Document Evaluation will be $530. The fee for the January 2010 Pharmacists Evaluating Examination is CAD$515.

You can now print out copies of the PEBC Pharmacists Document Evaluation Information Booklet, Application Form and Application Form Example. Adobe Reader is required which is a FREE program.
