Please note: There are two separate application forms and two separate fees for Pharmacist Document Evaluation and the Pharmacist Evaluating Examination. The fee for the Pharmacist Document Evaluation is $530. The fee for the Pharmacist Evaluating Examination is CAD$515. You can now print out copies of the PEBC Pharmacist Evaluating Examination Booklet, Syllabus, Application Form and Application Form Example. Adobe Reader is required which is a FREE program.
